I've been to Canada, beautiful place, good health insurance program, friendly people. If I were going to choose some place more exotic, I'd go for Belize. Warm climate, beautiful scenery and it's part of the British Commonwealth (therefore, they speak English).

Puerto Rico might be nice, too. However, they're technically part of the US, so that doesn't qualify for this question.
